    I don't think it's possible to only run cpm ads. You will have to run ppc and cpm ads at the same time (ppc is for text ads and cpm is for graphic ads), not sure thought.

    BurstMedia pays fantastic, but their fill rate is low - about 40% or lower. Their CPM rate is however above $1. So you get about $0.40 eCPM. Still very good.

    Use CPALead, visitors have to complete a survey before watching a video, you get paid for each one, there is a huge thread on CPALead in the Affiliates section.
