Looking for a PHP "love quiz prank" script

I'm looking for a script that performs a function similar to this site:

It is basically a prank site that enables the user to enter their email and get a custom link to distribute to friends. This link goes to a fake love quiz which their friends will happily complete, not realizing until the end that their personal answers have been emailed back to the first person.

Anyone know if this script or something similar is available commercially?
