Is anyone using a configurable contact form for their (self hosted) wordpress blog that will let you add fields such as Address, or any other custom fields ?

It should also hold up when in a post as well as a page.

    Any suggestions?

    Thanks

    I use emailme[dot]com ,

I just love the simplicity it give, we choose the option and we get the HTML code, after that...
We embed it in our page/post.
We can tweak it as we like.
Cforms is too heavy for me because it took so long to load in wordpress admin.
