looking for a form

I have a joomla site and am looking for a form that allows people to include message, my email, their email. Any suggestions?

You can write one in PHP or use the feedback form wizard

 

There is a contact page(with form) in joomla by default included .... also there is guest book extension .... search in link jams44 has given.
Else I can do for a small amount
