Revver, Blip.tv and AdBrite InVideo. The AdBrite InVideo link is in my sig. Revver is like YouTube, you upload it and they host it and you have to use their player. AdBrite InVideo you have to use .flv format video and host it yourself. Blip.tv is for episodic content like a podcast only. Blip.tv hosts your video, you also have to use their player and they provide downloads too.

AdBrite in general offers PPC and CPM ads and they pay a decent amount. About 25 PPC and 60 cents CPM in my experience. Good luck!

It looks like you use their ad-serving player interface, but you put it on top of your FLV videos.

Google AdSense for Video, which I'm not familiar with, offers CPM and PPC video ads.

Hmm... what requirements are there of yours? I know REVVER pays very well. You upload the video and use their player YouTube style. If you can't do that, I don't know what to do. Look for preroll ads.
