Landing Page confusion

    I have 12 campaigns in my Adwords account. Now I am creating a landing page but m confuse, should I create separate page for each campaign or just one for all as there is just a request form for visitor.
    please suggest me Webmaster fellows.
    And also tell me the header & footer info of landing page

    You have 12 campaigns to the same URL?... Shouldn't your campaigns be adgroups then?..

    Maybe you are referring to 12 Ad groups to the same url? I find it strange that you are creating 12 campaigns to the same website.

    Not necessarily - if you want different groups of keywords to have different targetting, or different budgets, this is quite normal.

    To take an extreme example, consider a big department store's websites. They sell everything from clothes to computers, but the different departments all have their own budgets.

    Or a company that has car dealerships across the country, but wants to bid on the same keywords in different areas, with different advert text (e.g. Buy a New Ford In XXXXX)

    Unfortunately, this also indicates the answer to the first question. The department store would want different landing pages, the car company may not (as long as the appropriate location was clearly visible on the page).

    If people are looking for different things, they should have different landing pages. If they want the same thing, then it's probably not necessary in general.

    Thank you CustardMite for your answer and please also give me some information about what things I include in footer of Landing Page.

    I go through many landing page but some of those have not included more option except.. Desclaimer, Trade MArk Policy, Terms & Conditions.........

    So Please suggest me should I use these options or not in footer of landing page
