    What "keyword density" really means?

It means the % of times your keyword appears in the html of the page.

A wouldn't get too bogged down in worrying about this.

If you stick to a 2-4% KW density per article/page, you should be fine.

I typically use my keyword 3-4 times per 300-400 word article.

Keyword density is based on how many times that word shows up in site. If you have it 4 times in a 100 words that would be 4%. Most people stay around 2 to 4% if you go to high they will consider it keyword spam
