Buying Javascript Popup Contact Form

We are looking for someone to create a “popup tips email form”, very similar (okay exactly) to the one on http://techcrunch.com like the one on TC, the user will click on a button/image to open form.

We are looking to do this right away within the next day or two. You can send us a PM or reply to this post and we will get right back to you.

Needs:
Title - text area
Message – text area
No need for users email address
Form closes upon send submittal or user can click close image/text if not wanted to submit anything
Would like small javascript text window thanking user for their tip and an error massage if cannot send message
Title and Message will be sent to a specific email address
We will provide all the text/images
We have a form on our site currently that stopped working; we can forward the files of what is needed (if necessary)

Requirements:
Must work on vBulletin 4.12
Must work on the Big 3 browser (FF, Chrome, & IE)
We prefer someone on Pacific Standard Time or East Coast Time
Skype is preferred after job is awarded, but not necessary
Payment will be made after testing on forum and using different browsers
