Why would you bother? If you've been banned twice, it's almost certain that you're going to be caught and banned a third time since it doesn't look like you've learned from whatever mistakes you've made. Instead, take up golf.

i guess you cant apply anymore cause the google will see your using the same Ip address and if youll gonna go to an internet cafe,, sooner or later they will know that its still you because of the same given adress that youll gonna you
you can apply with new website..just try. however there is no guarantee for approval..it all depends on the adsense team..
I think google do not trace ip. Because ISP cannot give static IP to all users (at least not in India). What I think is that you have chance to apply, if you have a site 6 months old which has not use adsense at all. Give new payee name. Give new address. Have a google account which is also 6 months old.
