Is this squeeze page adwords-friendly?

    I understand that Google hates squeeze pages with no content on it other than an opt-in form. I have set up a squeeze page with content using the navigation bar. In the main page, however, I only provide a video an an opt-in form.

    Now, I would like to know whether putting my content on the navigation tab is enough for Adwords to approve my squeeze page?

    You page is okay but I will suggest you to add your contact no and physical address near Form.
    Once, exactly similar account was suspended and then adwords executive told me to show other contact option too. Then it got approved

    I am curious on whether or not your page was approved. I worked on the AdWords Support Team but was not a policy specialist. I did not see proactol on the restricted list but I think you would run into problems with a lack of original content. Animesh provided some solid tips I think but I know Google puts a lot of emphasis on content and the video is the only content on the home page so you might have had problems getting approved. Again, I would be interested in knowing what result you had.
