Depends. If you only have 1 or two websites, usually the entire account. A person with ten-thirty websites, google will just ban the specific site. This is only applicable for things like "image-next to add", "html injection" and small problems. If we are talking about click-fraud or large issues, then it is a ban for the entire account. Peace,

I have had a friend who was earning some few K's a year from his game sites but he was banned several times because somebody has been false clicking.. now if his account got closed down how is he able to make another account? They banned the account so that you cannot continue with google? Can he use the same postal address when he signs up for another account or is that disallowed also?
Not possible. Google will never ban you because someone clicked on your add a million times. They have a special algorithm to detect if the click originated from the same person (more than just the IP). You are better off using a different address/name. Peace,

IP doesn't play in the equation, as long as it is under different entity (name/address), you can even insert it on sites owned by the banned user under that IP. Peace,
