The guy is real and is a bodybuilder, they are his muscles but of course photoshopped. Theres many bodybuilding photos around like this for fun. But saying that, current My Olympia level bodybuilders are generally beginning to look more cartoon like every few years. Look for pics of Ronnie Coleman, his genuine pics show his true freaky size. Added: Real and realistic.
The picture might be fake, but if you inject steroids straight into your muscles, rather than into your veins, you could look just like this guy. I still think its fake though.
I think if steriod is entered in veins directly, then it may kill a person. But the person in the picture is looking too healthy. Ya! It is a fake picture infact it's a good work.
Unsure if they would kill but you're correct that they dont get injected into the vein, they are intramuscular injections, so anywhere that theres some body mass deep enough to inject into but usually the buttocks.
