IP:Port Proxy help

I need to create a proxy for my own personal use. I need it to be the type of proxy where you enter your IP:port into your IE settings and then all traffic just goes right through it. Can anyone give me instructions on how to set this up on my web server or even just a link to instuctions?

I have a dedicated server and permission from the host to have proxies so that's not an issue and it will be exclusively for my personal use if that matters.

And before someone just posts a list of thousands of them available on the net already, I've used those and after a day or two I always get blocked because I use it to browse quite a bit and I assume they don't like me using up all their resources. lol.

I hope this is the right section for this. Thanks in advance for any assistance!
