Insurance Quote Request Forms and/or Lead Capture Forms for Independent Insurance Agency Website I'm needing custom HTML lead capture forms for multiple insurance types that once completed, forward to my email for follow up. My website is ran off Wordpress but we can use sub-domains if need be. Example: A customer clicks on a link for an auto insurance quote and is taken to the quote form page, they fill out their basic information, name, address, phone number, all drivers, driving history, vehicle type, coverage desired, current insurance, etc. Once the form is completed, it's emailed to my inbox for follow up. The forms must fit the color scheme of the site if need be and be somewhat appealing to enhance click through rates. Nothing too basic or bland. Each insurance type will have similar and different criteria needed to complete the quote form. I'll provide a list of needed content structure and examples as well for each type. Basic info would be name, address, phone, coverage desired, current insurance info, claims history, ticket history, illness', etc. The difference in content is somewhat obvious depending on the insurance type.
