    I'm needing a quote banner designed for an insurance site. The winning entry will receive $15 PayPal and a first chance to design the logo for an extra fee. You can PM entry or post below.

    Template Screenshot - Image will replace grey box

    Example of Quote Box

    What's Needed in the Quote box.
    - Size: 500 x 335
    - Matches overall with theme, stock images ok if royalty free.
    - 1. Compare Free Quotes From Multiple Agents
    - 2. Zip Code Box
    - 3. Type of Insurance box (Auto Insurance, Home Insurance, Life Insurance, etc.)
    - 4. Radio Button saying: Currently Insured? Yes No
    - 5. Button: Compare Free Quotes

    Format PSD, PNG, GIF, JPG

    I'll also discuss a creation of a logo at an extra fee with winner.
