If you're running a cPanel based server, you might want to ask your host to run a strict scan on their server using ClamAV. This might help in removing the Iframe injections not only on your account but also on others' account.

I have personal experience in this.

How to remove: if you just have a few web pages, simply open the source code and remove the bad code. If they damaged your codes then you would need to restore from backup. If you have so many web pages, you can follow Matt's advice, ask your host for help. They can make a script to auto remove the bad codes.

How the iframe got injected: they logged into your account via FTP => download the files => inject => reupload the files.

To permanently stop the problem, you would change your account password.

    Hope this helps :)

My all sites attacked by this iframe.

    1) remove all the bad codes from your html
    2) upload the all fresh files into your server
    3) Change your FTP user name and password
    4) don't save your password in the FTP
    5) Run a complete scan on your PC

Remove the iframe injection from the infected file and upload the new files. Plus, change ftp details for the website.
