The design is not bad, but in my opinion, to put the text "GET ACCESS TO ANY BLOCKED SITES AT SCHOOL OR WORK!" and not the proxy form directly can be confused for the visitors, you can increase the CTR because some visitors will not found any form and will click in the ads or any other click, but, these people will be all not returning visitors.

Design wise I think it looks great. I would add more text if possible. Also, for your adsense font colors, maybe make it black (the description) so its more visible?

Hi - Like the header/logo; nice!

Not keen on all the ads, perhaps reduce them slightly. Maybe remove the ones that do not make you money
