I urgently need the assistence of someone fluent in Arabic

Hello.

I know this is off topic, but because of the large number of Islam/Christian debates here, I suspect this may be the best section to find somebody who can read/write Arabic. (I apologize if that is more stereotyping than I think)

I have two forms which are in Arabic that I wish to understand - is there anyone here who would be willing to translate them for me? Please post here or PM me if you are willing.
