I urgently need a PHP contact script.

Hi,

I am a newbie to PHP and I need a contact form which the user can fill in their name and email address and upload a file that will be sent as an attachment to my email.

Things that it must do:

The email that I receive must appear to be sent by the email address the user entered.

I need to be able to limit the file type and the file size that can be sent.

I also need to be able to make any input field required that the user enter.

Like I said I'm a real newb to PHP so any help is appreciated.

I would really like a step by step guide of what I have to do.
