i need a web based contact form

    Hi everybody,
    I am in need of a server based contact form.
    I had one, but for some reason, i somehow f... it up (no clue how, it just won't work anymore) and i've tried everything to repair it myself.
    anyways, i've more or a less given up on that one, so i need a new one for a project i'm working on www.fly2asia.co.il

    i will translate it to Hebrew myself later on.
    Does anybody know where i can find one, or have one maid? i am not looking for those sites that manage your forms for you for a monthly fee.

    the fields i need :
    name
    telephone
    email
    departure date (with calendar date picking script)
    return date (with calendar...)
    drop down menu to choose from various products
    remarks

    please please help me out on this one or make me an offer

    PHP:
    The script assumes that the submit button is named "submit", and the email field is named "email". There's no validation, but I assume you can figure out how to do that... Point the form at the script with <form action="script/location.php" method="post">. If you're like me, you'll point it at the same page the form is on.

    You're right,
    i hate dealing with this and don't want to do it anymore.
    if someone can make a quick $ out of it and i can save time , why not...?
