Buying I need a squeeze page created

    I need a squeeze page set up, I have an example of the page I want, the first page I just want the person to enter in a few fields and then it goes to a second page where they enter in full details I need from them.
    This is simple for someone with the skills. Let me know how much you charge, lowest price wins.

    Will you provide the backend script (php+MySQL) of the site? Or does the coder have to prepare it as well?
