I need a simple PHP form for my website

I am looking for someone that can provide me with the code/HTML I would need for a simple contact form for my website.

I would need two pages. The first being the form. I would give you the fields I need. The second page would need to be the "sendmail.php" page that after the user submits would go to. This form would need to email the results to my email.

If you are interested, please reply with your cost for this service and any examples of work you can provide me and an estimation of how long it would take you to complete this project. I'm sure this wouldn't take someone with experience more than 15 minutes.

You are right using the sendmail function and sending mail to any email address wont take more than 15minutes. But designing form can take some time.
