I am in need of a simple website that has a 2 page form mail set up on it with an agree to terms checkbox. The site will be just a 2 column layout with very basic graphics which I will add. I have an example if you need one. Please PM me if you can do this job and are familiar with form mail. I will pay upon completion. I can do this job myself but it would take some time. If the price range is not good for me I will move on to the next person.
