    I need a form that allows for one of donations and recurring donations via PayPal, basically I need exactly what this site has

    Are you looking for a quote on a three-step form, exactly as shown in your example? Same fields as well? DO you need the HTML and the server code to process the docnation too? If you are accepting cards that would involve a merchant account or an integration with an online payment processor.
