Buying I need a Form Design (USA Designers Only)

    I need someone to design a form identical to another site we our partnered with. I'll pay you $75 dollars if you can make it almost identical and as clean.

    Yes it's simple though... I can install the validation for the form.

    Do you have a few websites you can show me first? We might be interested in having you do more projects we just need to make sure you have a sense for what we are looking for.

    Because we don't outsource and besides that we need someone that is from USA; therefore they can relate to things much better.
