i need a form and email script

i want to add a form in my website. please tell me how to make form and how to get it info via mail

You can add forms using <form></form> html
You can get info via mail by using mail(); function of php

 

Here's one I used a while back, after a little modification:

It's quit cool.

I have written script like that, so I will install it for 10$

 

please send me a form having 5-6 fields like name, email, address, phone no, message and php mail script...

It's as safe and updated as a script is going to get.
You can also add as many fields as you want and chose the option to validate.
