I need a custom icon for a windows application

    I need an icon for a Windows program I am developing. I would prefer to receive the .ico file already created with all of the sizes/color depths included.

    If you can't do that part, then I will need it in the following sizes:

256×256
48×48
32×32
16×16

And I will create the various color depths and assemble the .ico file myself.
    Code (markup):
    The program is Futoshiki Puzzles. Futoshiki is similar to Sudoku. My web site is here:
