I need a CSS styled form coded - $$$$

    I have a travel booking widget that has been redesigned and I need to update the form design.

    I don't care if it uses tables but the code has to be clean and match the style exactly.

    I will provide a PNG/PSD of the new form design along with the code from the old widget.

    Post or PM with offers, quotes or questions. I will be looking for examples of past work.

    Can be done for around 100-150 dollars depending on how complex it is.
    Will be valid CSS.

    Just a simple form. I'll take care of all the back end magic.

    Most complicated part is a drop down that appears based on what the user enters in the prior field.

    I have not yet received any reasonable offers for the task so the job is still open.
