I need a contact form built into the side of my wordpress blog

I have a wordpress blog with the "connections" theme installed. I've hacked out all of the sidebar except for the "about" + "links" and "categories" sections.

I need a to add a seperate box for a contact form which goes on top of all the other boxes which gets sent to email addresses of my choosing. Needs to match the whole theme and other sidebar boxes.

Form should have roughly all of the info below:

I need someone to create some code for me where I can just drop it in/customize the settings.
