How would you integrate affiliate link into a contest?

So I would like to have a contest on my site and I would give away something like an ipod to one lucky person in exchange for people filling out the affiliate offer. Of course that would be an incentive offer meaning completely legal to do.

Is there a WP plugin or a script that would allow me to do that?

Well I have a few accounts, clickbank, maxbounty, azoogle and some other that I can't remember at the moment. All the accounts allov me to do incentive promotions.
