Locking down web servers involves many factors

    1. Did your hosting provider admin put your server in secure sandbox such as, chroot, jail
    2. Have your web applications been checked for vulnerabilities?

There are many possibilities for viruses infections.

Sometimes, FTP passwords are stolen by trojan horses.
Sometimes, web servers are configured improperly and malicious user can hijack/modified files of others.
    ..etc.

Depending on the type of server your running, make sure that file permissions and directory permissions are correct so that files can not be added or altered by anyone other than user and root.

    May want to consider suPHP.

If you run a dedicated server, the options are endless; however there is such thing the security world as too secure.

If u have root access, just install mcafee enterprise, config it , then configure IPsec bock all traffic from outside and also inside and open necessary ports
