Get lots of backlinks, that matters more than on-page, anyway.

Its not that easy if your keyword has lots of competition.
Like webcosmo said, Google Webmasters Guide is a good resource of information.

Put your keyword in your title and your description, it also helps to have it in your url if possible.
Wrap your keyword in an H1 tag in your header.
Write your page copy to incorporate your keyword a few times.
Use that keyword as your anchor text in your backlinks from other sites.

You optimize keywords by choosing words and phrases that have a high demand with relatively little competition. If you've ever done manual keyword research, you know how time-consuming it can be. To speed up the process, you can use software and services to do the research for you.

to optimize a word you need to make the density high for this word. you can do this by increasing the number of occurence for this word. also put links that contains this word in the anchor text. put backlinks on other sites that have the word in the anchor text

It's not the count that's important. It's more the location and spread of them. Having them in the title, url, <hx> tags, and in the first 200 content characters all help as well as towards the end of the copyright to reinforce the keyword. It's also good to have keywords around relevant links and in the meta description. How often they appear in the content depends on how long it is. If it's less than 100 words then two or three time's is fine. Any longer aim for around 2-4%.

Most of my articles are 350-550 words in length, and my keywords are in h1-h3 tags, strong tags, and links. I scatter them through my regular paragraphs as well so Google doesn't think I'm gaming on specific keywords, but you should repeat them throughout your article (not to the point of overkill, though).

Keywords are not as critical as they once were. However my advice would be to use your keyword in your main webpage title and then a few times throughout the content. i.e if your keyword was "widgets" use a page name like how-to-use-widgets.html or how-to-use-widgets.php, then use a <h2> tag with the title i.e <h2> How To Use Widgets </h2> then use the widget word throughout your body of content, but don't go crazy and stuff your article with the keyword. Try to use the word widget in the first and last line of your content

sfraize has written an excellent, short summary answer to the OP's question. If you haven't read it, you should.

1 thing to be careful of is not to target 2 pages on your site with the same keyword, you end of fighting with yourself

Yeah, I think one key word to 100 words is enough.
