How to make/implement Proxy Templates

I am looking to start a proxy website. I have (what I consider to be) good skills in photoshop. do people just design a 600x800 image and use that as a template? that seems like it would be a big file that would take a long time to load... the only reason I ask this is because I see people selling PSD files.

Can anyone tell me the step by step directions to making and implementing one of these templates? or possibly point me to a website? or anything in the right direction for that matter? Thanks
