How to make contact us page?

I'm working on a blog in Wordpress and need to make a 'contact us' page, where they enter information and it gets forwarded to my email, but I have no clue how.

Any help (or even basic script) would be appreciated. Thank you.

It's a contact form w/ spam protection. I don't know php so I'm not sure about security, but I know the plugin works.
