How to insert Clickbank ID's into affiliate tools?

    Hi everyone,
    I am trying to figure out a way to add Clickbank ID into affiliate tools (banners, text links, e-mails etc) for affiliate page.

    I assume there must be javascript for this (getElementById ??) but not sure. If someone can help me with this I will greatly appreciate it!

    What I need is the form under wording "Insert your ClickBank "nickname" in the FORM below, click submit, and your links will be customized automatically"
