Download my Adwords FAQ for more detailed information. There's a section there about your specific question.

Obviously, your ad needs to rank in the top three. You also need a QS threshold so even if Google calculates your ad as being ranked in the top three, if your QS is not good enough, you'll be pushed down. There could be no ads that meet the requirements so you won't always see three ads up there. There could be none, one, two or three.

You're probably just not bidding high enough. Like Lucid said, you need to be in the top three search results to show up in the yellow bar.

To solve your problem about not seeing ads, I'd recommend using the Google Ad Preview tool at http://www.google.com/adpreview. You can get better ad diagnostics using this rather than just running Google searches on your own. Also, keep in mind that just because you're bidding on keywords, it doesn't guarantee your ad will show up on 100% of searches - things like budget, bid, and quality score all play a role in how often your ad shows up.

Bingo. Depending on the keywords you are bidding on, you'll more than likely pay a pretty penny to see your ad there.

No, Lucid web marketing had it right. It has nothing to do with how much you bid. You can bid as high as you like, $100 if you will, and achieve 1st position but that can still be on the right hand side. The top left spots are reserved for advertisers by Google that have exceptional Quality Score and account history, and it is not just a place for those with the most money to bid on.
