It is not strictly determined. From my observation frequency is about one time per 2-3 months. Sometimes 1.5 month, sometimes 4. But I also found that Google reindexes pages and updates PR values more often for the most "popular" websites.
content is in no way connected to page rank .you need your blog link to be present in other top sites or atleast medicre blogs - but yeah if your content is great and unique there are possibilities that they will be automatically included in other blogs along with your blog link .

try to get inbound link because links will improve your overall page rank. it may not make a difference overnight, but it will make a big difference in time.

Ignore what half of the guys posted above. I mean, don't really ignore them, but the "get links, many links, as many links as possible" approach is stupid and not that helpful anymore. Nowadays, social media activity seems to weigh a lot in search engine placement, and could affect the pagerank too. Try to get on as many social networks as possible and make people share your content (without spamming in any way). It'll go naturally from there.

You should put all your focus on getting high-quality back links in order to establish the authority of your site. Once you have that, your site can automatically have a high PR that you want. But, of course this is easier said than done. You must spend a long time building your site's authority online.
