how to Create a Contact Form in Picture

    hi alli created a web by photoshop I saved the files at htmli opened them in dreamweaverbut i want to create a contact form on imagelike this web josephones.com (Bottom left corner)i have the background(the image that i created on photoshop)but How can I place the elements at desired me on the image?I hope I explained myself correctly I will be happy to help

    HiThanks for the quick responseI'm already did it ( somebody do that for me but i'm steel dont know how :/ you can send me a guide or something?)but i cant to write text on the imageIt's writting out of the image
