How to change wordpress post tittle URL

Currently my post tittle URL is working as

http://www.usgadgetonline.com/index.php/2011/11/quran-with-tafseer/

I want to change it like http://www.usgadgetonline.com/quran-with-tafseer

Does not find any default option to do that. how to do that

 

Change permalinks to /%postname%/

Change it and put..... in custom field:

/%postname%/
