If you want to host it on your own server you'll require a domain and hosting. For this you can use a domain registrar, such as:

GoDaddy
NameCheap

And a hosting provider such as but not limited to:

HostGator
1+1

Upon purchasing a domain you'll point it to the DNS of your hosting account, then you'll add the domain through your hosting control panel (Likely cPanel on these hosts) which will then allow you to upload your dreamweaver template files through an FTP client directly to your domain.
