Some themes come with pre-existing ad locations. If you dont have these then they are several plugins on the wordpress repo that will allow you to choose locations within your page to display adds. This link gives me 125 pages worth of options: http://wordpress.org/extend/plugins/search.php?q=ads

Plugin is a good choice, or you can hire somebody do it, place the ads at any place you want, it's easy.

You can use plugins or manually edit the wp files.

You can paste the code for your Adsense Ad unit anywhere you choose - but it's messy and not something visitors want to read.
As already explained, you can add the code to text widgets or anywhere in pages or posts you write yourself.

Some themes and plugins allow you to simply enter your Adsense publisher ID once - and they take care of displaying ads of appropriate sizes in selected locations.

If you've set up your wordpress permalinks correctly, then your post title becomes part of a url - to help seo ranking.
So you don't want to put ads actually in your post titles.

I guess most people are more comfortable with using plugins, however I would simply edit the code -_-
