I'd personally lose the reset button - for form that size, hitting the wrong button can wipe out five minutes work. How often do you fill a form in then want to reset it anyway? I can't think of a single occasion when reset has been required. If you want to amend a field, its normally just that - one field.

I'd loose the reset button for the same reasons SEOWOES said. If you want to reset it, you can just delete everything yourself.

I agree with the above. No reset button is better. But if you just HAVE TO HAVE ONE, you should definitely have the "Submit" button FIRST and the "Reset" button far from the "Submit" button to avoid accidental clicks.
