    search in your wordpress admin panel for "contact us" in the plugin section and you will find one easily.. contact form 7 is pretty good, i've used it before. although, if you want something quick, then maybe use something different.. contact form 7 has a little bit of customizations you can do to it, so it may take you 10 or 15min to learn it.. so not that QUICK.. but still pretty quick .. ;)

    It is very easy to create a contact form in HTML using HTML tags.first, you can create a table which have tr ot td tags.
