1 or 2 high volume / competition keywords and unlimited long tail keywords, basically as much as i can get in the content of given webpage. I don't do anything specially for those long tail keywords, they will rank themselves, i am focusing on those couple high volume keywords.
There are always a few big terms you want to rank for, but the majority of your traffic will come from the long tail keywords.
As many variations as you can. Your not going to rank for every keyword but its easier to rank for low competion keywords

Anything over 400,000 is going to need more than an exact match domain to rank. Meaning, if you type in "weight loss" Google will show About 1,130,000,000 results. If on the other hand you search for "premier periodontics lexington ky" you will see About 52,900 results. To rank for the latter you can pretty much just buy a domain like Premierperiodonticslexington.com and put up a WP blog, do a few citations and social media profiles and that will be more than enough.

First, I'll search keywords for my website and find out their search volume and competition. I can target 3-4 keywords according to their search volume and competition. If for one keyword my website ranks, that's good for me. Then I'll rank other keywords.
If monthly search volume is also considered, one should at least target 3 keywords per page. Divide them into Long-tail keywords and other high-ranking ones. Having multiple long-tail keywords is definitely gonna help.
I wont target for keywords. Keywords can be specified but should not be done with intention. It should look naturally and informative.
