How I get readymade form submit script including mysql database query ?

    That's too vague, you need to give details on what you want. Do you want the form to POST or GET? What are your MySQL database fields? What is this script supposed to do?

    You mean you need to edit DB
    Just use phpMyAdmin

    Talk to your employer about the gaps in your knowledge. Obviously you didn't cover PHP as part of your degree and they will/should know that. They need to understand that for a while your productivity will be lower and to bill the client accordingly (ie lower the charge out rate).

    In the meantime search up PHP tuturials, however, most people these days are using a framework or a CMS to how they build forms is dictated by that. Few of us are building html forms from scratch.
