I have 4 or 5 different usernames and passwords that I use for most all of my stuff depending on the requirements as well as the importance of the account to me. For example, just a forum I sign up on where if it gets stolen or compromised isn't htat big of a deal then i'll use one password, my more important stuff like paypal has a more secure one.
I use the old fashioned way, paper I don't really have many places to use a password, maybe ten in total, and I can remember the info I need to after a few days of typing it in.
Most of my passwords are saved into my brain as well as their usernames. Only rarely do they ever conflict but when they do it gets annoying trying to figure out which password goes where. Secure though, I doubt hypnosis would be able to get it out of me.

SpashID on my desktop > mirrored to my PDA. I have about 400 logins/passwords, so keeping them memorized just ain't going to happen.
I store them on my PDA using an app called SplashID. It is designed for storing passwords and just about anything you could imagine from bank account numbers to lock combinations.
