Those are triggered when you are in the geographic location that the advertiser targets. From your examples, both those advertisers are geo-targeting their ads, the bottom one probably only to the city of Sydney and the top one to the whole state of NSW. I take it you live in Sydney. Knowing this and the geo-targeting used by the advertisers, Google made the ads more attractive by displaying the city/state.
What happens if you custom set your geo-targeting to for example 50km around Sydney, would it still show up as "Sydney, NSW"?
