how do you create phrase match or exact match keywords?

    I'm not sure which is which:

    "keyword keyword"

    [keyword keyword]

    keyword+keyword

    I have thousands of keywords should I make exact and phrase matches with all keywords does it help that much?

    "keyword" = phrase
    [keyword] = exact

    what's the difference between a "keyword" and a [keyword] ? I figured a "keyword" is also an exact match?
