how do you create exact or phrase matches on ppc campaigns?

    I'm working on my ppc campaign and need to upload keywords that are exact matches or phrase matches how do you do that?

    Is it like this: keyword+keyword or "keyword keyword" or [keyword keyword]

    Examples:
    Phrase Match = "blue widgets"
    Exact match = [blue widgets]
    Broad Match = blue widgets
