How do i link up a contact form in flash?

    I am creating a website for my fiance but on the contact us page, how do i get it to submit the form in flash? I have cs5. Right now the form doesn't work cause i am editing a template. How do i get it to work?. I attached a pictured of a screenshot on my mac. Help!
